CONSULTING AND THERAPY SERVICE AGREEMENT
Our service agreement outlines the expectations of all parties, details what the hired service involves, and other important information like location, resources, and payment information.
We will go over the agreement details with each client, and once they have been read and understood, the contract will be signed before services begin.
CONFIDENTIALITY
Confidentiality means that the Psychologist-Consultant cannot provide any information about you to another person. However, there are some exceptions to this rule, which are as follows:
1. Your Psychologist-Consultant believes that your life or the life of others is in danger.
2. Your Psychologist-Consultant must write reports about their clients to the agency that funds the therapeutic or consulting service; however, your written permission will be requested.
3. Your Psychologist-Consultant undergoes professional supervision. Professional supervision means that your therapist meets with another qualified therapist to discuss the work they are doing with clients. However, the supervision itself is confidential, and in most cases, information identifying you will not be provided to the supervisor.
CANCELLATION POLICY FOR APPOINTMENTS
If you are unable to attend your appointment, please notify us as soon as possible so that we can reschedule it. Any missed appointment for which we have not been notified at least 24 hours in advance (24 hours before the start time of your appointment) will incur the full fee.
Rates/Investment
Each client has unique needs and expectations; therefore, we carry out an initial FREE assessment to gather information, then an intervention program or a series of sessions with the client(s) is agreed upon.
Once agreed with the client, payment is made by credit card or direct debit at the time of registering for the chosen consultation. You will find the options for different consultations in the “prices/consultations” section of this website or click directly on the button below.
